Creating a Social Worker Referral

To create a Social Worker Referral follow the steps below.

1.  Select a student name from the Student Card or by using the Search Student box.

        a.      When using the student search, enter the first 3 letters of the student’s last name in the box and search



2.  Selecting the student name to go to the Student Profile.  Select the “New Social Worker Referral” to begin.



3. Complete the form:
         a.      Select the appropriate Presenting Issue(s).  Multiple Presenting issues can be selected for a student.
         b.      Enter in a description of the concerns. 
         c.      If additional details are needed, enter them in the Additional Details text box.


       d.      *Required: Select actions you took prior to creating a referral and enter the date and time of the action.
       e.      Indicate the source of the referral if you are not the source by selecting the appropriate box.
       f.       Submit the referral.

Child Abuse Referral



If the presenting issue is “Child Abuse/Suspected Child Abuse” you must acknowledge you completed the proper form to report the issue to DHR.

1.   Select the check box “I have completed DHR Report Form 1593 and submitted it to DHR via __Email; __Fax.



2.  Indicate how you communicated the report to DHR and log the time and date that you communicated the form.

3. Select which option you want:

     a.      Submit Referral to notify Social Worker:  This option will complete the form and notify your school assigned Social Worker/Workers of the referral submission.  Social Workers will get an Alert for cases involved with Child Abuse/Suspected Child Abuse. 



        b.      Add Additional Presenting Issue:  If you have additional presenting issues to select in addition to the Child Abuse/Suspected Child Abuse then select this option and you will be able to add additional presenting issues to the referral.

Note:  If you do not complete the Alert acknowledgement, a notification will still be sent to the school assigned Social Worker or Workers.  Social Workers will reach out to as to why the process was started but not completed regarding the student.

4.   Referrals created by you can be viewed on your workspace in the “My Reports” section.
